Notice of Privacy Practices

This notice describes how medical information about your child may be used and disclosed and how you can get access to this information. Please review it carefully.
Fayetteville Children’s Clinic is required by law to maintain the privacy of your child’s protected health information (PHI), to give you this notice of our legal duties and privacy practices, and to follow the terms of the notice currently in effect.
How we may use and disclose health information
Treatment: we use your child’s health information to provide care, and we may share it with specialists, hospitals, labs, and pharmacies involved in that care. For example, when we refer your child to a specialist, we forward the medical history the specialist needs.
Payment: we may use and share health information to bill your insurance company and process payments, such as confirming coverage or providing the information a plan needs to pay a claim.
Health care operations: we may use health information to run the practice, improve quality of care, and train the medical students and residents who learn at our clinic under supervision.
Other permitted uses: appointment reminders by phone or text; informing you of treatment options or services; disclosures required by law, for public health (such as immunization registries and disease reporting to the State of North Carolina), to prevent a serious threat to health or safety, for health oversight, in response to lawful court orders, and to coroners or law enforcement as authorized by law.
Any use or disclosure not described in this notice will be made only with your written authorization, which you may revoke at any time in writing. Uses such as marketing or the sale of health information require your authorization and are not practices of this clinic.
Your rights regarding your child’s health information
You have the right to inspect and receive a copy of your child’s medical record, to request corrections, to request an accounting of certain disclosures, to request restrictions on certain uses (we are not always required to agree, except where the law requires), to request confidential communications by alternative means or locations, to receive a paper copy of this notice at any time, and to be notified if a breach of unsecured PHI affects your child’s information.

Minors and parents
As a pediatric practice, we generally release a minor child’s information to a parent or legal guardian. North Carolina law gives minors confidentiality rights for certain types of care; where those rights apply, we follow the law.
Changes to this notice
We reserve the right to change this notice and to make the new notice effective for all health information we maintain. The current notice is posted on this website and available on paper at the front desk.
Complaints
If you believe your child’s privacy rights have been violated, you may file a complaint with our office by calling 910-484-3121, or with the U.S. Department of Health and Human Services Office for Civil Rights. You will not be penalized or retaliated against for filing a complaint.
